Focus 2001 Coolant Problems

I have been having problems with my 2001 focus. the coolant level had dropped very low and I had to top up 2 litres.  The radiator fan comes on even in really cold weather, and steam comes out of the exhaust on high revs. 

 

Any suggestions?

Sounds like the head gasket has gone. Take the oil filler cap off to see if there is any mayonnaise looking gunk on the inside of the cap. Also check the oil level to see if its gone up. It may not have as the water evaporates.
