Has anyone else had a slave cylinder collaps and destroy the clutch and pressure plate?

Ive had a slave cylinder fail and leak fluid all over the clutch , contaminating it - on one car

 

and ive had the DMF break up and fill the starter motor with dust on my TDDI , both got treated to SMFs and uprated clutches
