Focus C Max fault at 2400 revs!!

Hi, I've recently bought a 2005 Focus C max 1,6 tdci with 108k on the clock It will start up and run great, the problem occurs when I set off and get to around 2400 revs the car will hesitate seriously & begin to  kangaroo down the road? It will drive fine on the road and motorway as long as I stay below 2400revs its driving me mad. Ive not had the car long enough to know how long this fault has been apparent.

I've recently changed the timing belt,water pump, bottom pulley, blanked EGR valve & replaced all the filters. I have noticed number 4 injector is chuffing slightly but I wouldn't of thought this would cause the problem. I cant see any split boost pipes either?

Has anyone else had a fault like this before? Any advise would be greatly appreciated!

If you put your foot down (from a standing start) do you get plumes of smoke?

 

Its possibly a split in the intercooler pipe, an issue I fixed on mine with great results. Take a look at my guides :)
