MK2 Focus 1.8 TDCi EGR Issue -- car stalling

Hello guys I'm new here so I'm sorry if I've posted this in a wrong section.
I have a very annoying (and sometimes even dangerous) issue with my focus.
Randomly my car stalls and bc shows "Engine Systems Failed" message on the screen. After I restart the car it works normally. It almost always happens when decelerating or when moving away from a light or a parking space.
This is the error message it stored in the memory: 
===PCM DTC P0489-61===
Code: P0489 - Exhaust Gas Recirculation Control Circuit Low

Additional Fault Symptom:
 - Signal is Above Maximum Threshold

Status: 
 - DTC Present at Time of Request
 - Malfunction Indicator Lamp is Off for this DTC

Module: Powertrain Control Module

Diagnostic Trouble Code details

Exhaust Gas Recirculation Control Circuit Low

This DTC may be caused by :

Wiring Harness Open Circuit

Short circuit in wiring or component.

Short to Ground

Exhaust Gas Recirculation Valve

Faulty or damaged PCM.

EGR control module was changed recently but the problem reappeared. inlet manifold was also cleaned.
Can anyone point me in the right direction to solve this problem because my focus turned itself off in a roundabout and in the middle of an intersection which could be very dangerous and i have a 1000+ km trip soon.
