Uneven power delivery Worst in 4th gear No DTC's

I have a 2011 Mk4 Mondeo 2.0 163 Tdci. I have an ongoing problem that is driving me crazy.

When Pulling away I get a really uneven power delivery. At around 3000 rpm's the rev counter will suddenly drop down to 2500 rpm then surge up back to normal as I go through the gears.

On motorways the car does not want to pull around 3000 rpm's and again the needle will drop then surge from 2500 rpm to 3000 rpm.

The ride is ***** awful and not smooth at all.

I have checked for errors using forscan and nothing is showing. 

When the engine is cold the car runs smoother but after 15 mins the problem returns. I have checked for cracked/broken hoses.

I plan on changing the fuel filter but I have a nagging feeling that this could be a Turbo Actuator problem.

The car pulls like a train once it gets over its little spasm around 2500-3000 rpm so I am hoping it is nothing serious.

My other thought was that it could be the sensor on the accelerator peddle but I am not 100 percent on this as it seems to be temperature related.

 

Any thought or advice would be very welcome
