Ford fiesta surging, why ?

My fiesta keeps revving up and down, when putting it in first gear the revs drop so low that if acceleration isn't applied it, the car just stutters and stops. Please see video link attached

does go away but it's like a tambourine noise. Sometimes as in the video it surges and reves up and down. I'm tired of this but I don't know what's the issue, I will clean the throttle body but it can't be that.

Only on really long drives the noise goes away but it's really irritating 

Guessing it's a diesel looking at that rev counter?

MAF and/or MAP figures should be checked on live data as I suspect one of those is wrong.

If it's not that then it's probably a fuelling issue, likely to be one or two dodgy injectors with the others trying to compensate.

As above and include fault codes, fuel rail demanded and actual pressures (if diesel) etc etc - any parameter that is odd?

Does @FordFiesta2011 know "We" recommend Forscan and lead as an alternative to just guessing or doing other random things?
