1999 Endura E Pcv System

I'm trying to figure out how this works on the 1999 endura e what I know:

PCV is located in filler cap

What I want to know-

Does the crank case breathe up through the rocker cover to the cap? if so I assume that the pipe that leads off to the manifold is exactly that, if I follow it, it splits off, two pipes appear to to go to the manifold and the third disappears down the back of the engine (Where does that go?) - It doesn't make sense to me that this tube is the crank vent.

How can I diagnose problems with it as when we bought the KA the filler cap was slugged up :(
