Juddering in high gears/under load

Ford Focus 1.5 Diesel is juddering a lot when accelerating in higher gears (6th, 5th and sometimes 4th). Starts fine and drives ok in the lower gears. It’s never failed to start and hot weather seems to make it worse. I’ve changed the fuel filter thinking it could be that but still the same result. Any ideas?
