Does the picture from forscan tell me that looking at the engine from front of the car that the 4th injector on the right is needing to be replaced?

Well it's not a great start.  Do you have any symptoms of a possible serious issue such as limp mode, loads more smoke than usual when booting it, poor idle or a misfire at low revs and low throttle input?

 

Although it's too early to tell you should be aware that Ford had an issue late 2008 through most of 2009 with a large substandard batch of injectors that were prone to wearing out far earlier than expected.  The issues a TSB (technical bulletin) regarding a software update but the only real cure was replacing the faulty injector(s) with a later version, somewhere on here there's a long thread that details the issue.
