Rough idle, lack of power issues, poor fuel economy

Hi all I've been having a problem for the past couple months with my Fiesta ST 180 Mk7.5 (No mods).

Car usually idles fine on cold start, but once warm it fluctuates. I feel a significant loss in power around 2500rpm and above, feels very flat. I can hear the turbo spooling but very slow acceleration. If I am full load on the throttle the engine hesitates momentarily cutting power, then proceeds to accelerate slowly. Also I'm getting really poor fuel economy.

I'm getting the following fault codes on FORScan:
P2108 - Throttle actuator A control module performance
P006A - MAP - Mass or volume air flow correlation
P2279 - Intake Air System Leak Bank 1

I've inspected the intake hoses, taken apart intake manifold and throttle body and replaced the gaskets and I can't find any signs of a leak. I've attached photos of data read for MAF, MAP, and IAT along with their voltages. I'm not sure whether these values are correct. The first image is during up and down idle (from 1000-1500rpm), and the second is on full throttle on dual carriageway. 

 

I'm pretty confused what the problem is since I can't find obvious signs of leaks. I've ordered a new MAF and MAP sensor in the meantime. Any help or suggestions pls go ahead, I'm at my wits end!
