Car radiator fan never spinning. MK 3 focus 1.0 eco boost

I noticed last week that the engine was over heating, It had melted the little square hard plastic on the bonnet sensor. I also noticed the coolant in the tank was bubbling. The raidiator fan doesn't work so I had my friend run a diagnostic test. It came back with error "fan 1 control module." 

I've replaced the fan control module, along with a new coolant temperature sensor and also a new coolant tank cap.

The raidiator fan still doesn't move at all. I've tried driving it and also being in idol with the A/C on. The top and bottom raidiator hose get hot so coolant is flowing ok. If I rev the engine and watch the coolant tank, I can see the coolant flooding in the coolant tank faster when I rev. So I think the water pump and thermostat may be ok. 

The only other point is that no matter how hot or cold that car gets, it seemes to warm up to 90 degrees on the dash and stay and 90. I've never seen it go above or below this. 

Ive also checked the fuses and relays in the engine bay. I swapped the horn relay for the cooling system one and still no joy.

Could the issue be the raidiator fan motor or a partly blocked raidiator.

The fan should kick in right away when A/C is switched on too cold setting. If it doesnt then id say fan is fubared

can I take out the radiator fan easy enough? I mean without taking the bumper off.

The gauge is driven by the ECU. It doesn't reflect the true temperature and is of little more use than a simple warning light.

Right, I've noticed something today. The fuse diagram is showing there are 3 relays labeled "cooling fan relay." Which are R4, R8 and R12. I only have a relay in the R12 location. Could this be the issue or are the other 2 relay locations not needed?

R4 and R8 are not used on you vehicle. The fues box does not even have any connections for those two relays.
