2016 transit connect wheel nuts coming loose!

Just a shot in the dark but.... 

I have a 2016 transit connect, 35k on the clock

I had the front pads changed and since then the wheeel nuts seem to be working loose enough to allow the dics to move (apparently on this model the discs are held only by the wheels!!) causing a clunk under braking. I've changed the nuts but still have to tighten a quarter of a turn every few days. 

I had the rears done yesterday, did a 90 mile trip and the same has happened there (no trouble prior). 

I watched the fitter tighten the nuts with a torque wrench to Ford spec (I advised him of the front issue prior so he avoided using an impact driver). 

The garage is baffled, anyone else had this issue?
