I know this is a subject that has been a problem for many and is a bit of a catch all warning for all sorts of problems, however, this may help someone. Having been getting this with a warm engine, no limp mode but a hesitation accompanied it; I tracked it down to the Rail Pressure Sensor wiring. The problem occurs because the cable that links the gear stick to the box presses on the wire to this sensor. If you remove the air box you will see the wire to the right (looking from the front) side coming off the injector wiring. It runs off the top and down the right side (looking from the front) of the engine to the sensor just below the EGR. Pull the gear change cable to the right and fasten it with a cable tie, make sure the connector on the sensor is firmly pushed home and if necessary repair any worn through sensor wires. Problem fixed and all for the price of a cable tie and possibly a bit of wire.
