What does the drilling sound from under the hood mean?

Hi all, a few months ago I had a strange noise sounding like a drill at construction side but it quickly disappeared. It then came back again for only a minute but today, while I was driving back home, the random noise reappeared and increased in strength. It continued for about 10-15 minutes as I was driving(only to disappear as I've shut the engine off). It only happened when I was driving and also when I was pressing the breaks, it did not disappear but instead gave out some metal grinding noises. I thought that it must have been something to do with the fan but turning off heating did not help.

Any ideas on what could that be and how serious is it?
