Somethings not right and I can’t quite figure it out

Hi guys, somethings not quite right with my 1.6 tdci 12 plate focus, the issue doesn’t occur when the engine is cold only when it’s warm, when I put my foot down at about 2k revs in any gear the rev counter shoots up to about 2.5k and holds there as If it’s trying to catch up then continues to accelerate. Also if I’m cruising on the motor way in 6th holding at about 2k revs the needle doesn’t stay still it kind of wobbles up and down. I can’t work out weather it could be clutch sticking or turbo issues, any ideas or advice would be greatly appreciated 



It would seem like the beginnings of the clutch friction plate slipping.
