Recently my car went into limp mode I had diagnostics done and apparently it was fuel injector 1. Iv had that replaced but now when you get between 2000 and 3000rpm and start to slow down a message comes up saying engine malfunction and it goes into limp mode, if you switch car off and back on again it’s out of limp mode until it happens again. 
 

If anyone has any ideas I would be very great full as it’s driving me insane

    I have the same problem with 2012 S-Max 1.6 TDI. Recently changed all injectors, with soon to have 300k km. Were you able to get your problems fixed?
