Limp Mode Issues - please help!

Hi all, I am having issues with my 2016 Transit Custom 2.0 Ecoblue (wetbelt) van, in regards to it throwing itself in limp mode. This is happening when travelling at slightly higher speeds (dual carriageways and motorways mainly), but only when there is a slight incline to the road. The van will throw itself into limp mode and flash ‘service required’ on the dash, until the incline stops and then will resume to normal with no lights on dash. On the plug-in computer, the error is saying something to do with injector 1 and 4 maybe, although all injectors look to have similar measurements.

For a bit of background, I had originally thought this was due to the egr being blanked and not mapped out, but I have recently changed the egr and it is no longer blanked. I also thought it could have been due to the van having high coolant pressure, but I have recently fixed that this weekend too, by way of head gasket change and new thermostat.

It is bothering me as this is now the only issue with the van, no error codes besides this.

Would be grateful for some advice or experience with this issue please.

Exactly what problem/code is indicated and what measurements are given (not that I'm an expert  on any of this but the more info the better)?

Is this with Forscan (best?) or generic (maybe not so best?)

If EGR was "mapped out" has any ECU change been corrected along with any other software "damage"?

Fuel rail pressure (demanded and actual) and DPF readings, plus other fuelling/EGR and turbo readings to look for clues and my usual... unless and even if it has been changed recently, change the fuel filter (boring though it may be!)
