MK4 2.0 EcoBlue Manual P127A during KOER

I'm hoping to get some advice on an issue with my MK4 Focus 2.0 EcoBlue Manual 135k KM. The engine is running quite rough and I'm experiencing some concerning symptoms.
Periodically, the engine develops a loud, harsh tractor sound, like an old diesel engine. The engine sometimes hesitates/stumbles when I steadily pressing the gas pedal , or I floor it.
Initially, I scanned the car and found the following DTC: P20EE:00 - SCR NOx Catalyst Efficiency Below Threshold (Bank 1) but after I cleared it , didnt come back. However, I suspected this might just be a secondary symptom of a bad combustion process rather than a faulty NOx sensor. I hooked up Forscan to dig deeper and tried to run a KOER (Key On Engine Running) On Demand Self Test.The KOER test immediately aborted and threw this code: P127A:00 - Aborted KOER - Fuel Pressure Failure.
My fuel filter currently has less than 30,000 km on it
I ran some live data in the forscan software but it all seams okey .
Any advice or suggestions?
