Tempremental Acceleration/revs. Help!

hi all :) i have a 2005 mondeo zetec 2.0 tdci 115. had the car for about 6 months and i am made up with it. Although recently having a few problems: main problem is the tempremental power of the car. it seems to lack in power and sometimes wont even rev fast enough when foot to the floor when in all gears. sometimes all is well (usually when engine is cold) but often, quite frankly it is running like a bag of s**t! Tried cleaning the egr valve but dosent seem to make any difference. could it be a sensor on the throttle? or is the turbo on its way out? Would be grateful of any advice. cheers.
