EGR valve with P0405, can't remove fault code

Hi all

I've got mondeo mk3 2.0 tdci with electronic EGR (black connector), and my problem is that at the beginning, my car was "farting" with black smoke around 2000 revs, it was accelerating slowly when reached 2000 revs black ***** from exhaust was coming out and after that car was accelerating brilliant. After few months EML light started showing up with fault code P0405, I've, cleaned EGR, inlet manifold still no result, so I've changed EGR twice first for brand new after market one, but car was very sluggish had big problem to accelerate and was smoking even more, second for genuine used one, problem was coming back smoke and P0405. FORDScan showing only that one fault. When I've checked on live data got information like this
EGRVP_V 1.95 Volt
EVP 0.00 Volt
EGRVR 78.12% but after 2-4min that value drops to 0% and in the pipe going to EGR suction disappears. But when I disconnect MAF sensor and start the car EGRVR showing only 28.17% that reading stays on even while reeving, but MAF fault code showing up, so after connecting MAF sensor back again resetting all fault codes and starting the car again EGRVR goes straight to 78.12% and after 2min drops to 0% stays like that all the time, and fault code showing up. 

Is is possible that wiring loom is faulty? Can someone check what are the voltage readings on the connector please so I could check with mine.

My cables are
1. yellow/blue
2. brown/blue
3. white/blue

 

any help or advice will be much appreciation.
