Ford Fiesta Intermittent Idle issues

Hi guys,

im having trouble with my Ford Fiesta Titanium 1.4TDCI and I’m at my wits end with this car. The car had an issue where it squeaked after turning the ignition off and I had the EGR valve replaced which fixed this issue. The car since I bought it 6 months ago has had an intermittent issue where the radiator fan kicks in, the engine revs itself at an idle and the car hiccups when driving. 
 

this happens when the car is cold or warm and keeps going until I restart the car or cool it down by putting the heaters on full with the windows open.

ive attached a copy of what came up with my scan tool (codes etc).

i use the car for work and need to get it right soon. Any advice or help is welcome.
