Inlet Manifold Failure - Opinion On Cause Please. Thanks.

Opinions please.

My Mondeo 2006 LX has suffered a failure in the inlet manifold. See picture (big chunk came off on cyclinder 1)

It has the 2.0 Duratec HE petrol engine.

Any opinions on the cause?

Failure in the Inlet Manifold or fundamental problem with the engine?

Should I repair or it's likely yo happen again?

    Its very comon problem most people remove the flaps and fill in the hole the rod gose through.Its a bit of a poor design factor really.
